KLAS Users' Conference 2005 - Shirt Order Form

To send in your shirt or blanket order, please fill out the fields below and use the submit button at the bottom of the screen to e-mail the form to Keystone. You may order up to 1 golf shirt per order submitted.

Checks should be made payable to Keystone Systems, Inc. Payment will be accepted by cash or check at the conference / upon delivery. If you are unable to attend the conference but would like to have a shirt please contact us to make arrangements for payment and delivery.


Golf Shirt Ordering Information:

 Willow Pointe - Striped Trim Pique Golf - 7 ounce 100% combed cotton pique golf with a three woodtone buttons contrasting clean finished placket, three color wide stipe collar & welt cuffs, single needle safety stiched shoulder seams & armholes and an extended tail with side vents. Color choices are White shirt with Mallard/Khaki/White Trim or Mallard shirt with Mallard/Khaki/White Trim. All shirts will be embroidered with the KLAS logo in a complimentary color.
